Registration Information
General Information: We will utilize this section to define terms and pass on information regarding:
1. Insurance Information 2. Co-payments 3. Inpatients 4. Outpatients 5. Prior-Authorization 6. Pre-Registration 7. Requirements for all registration
Insurance Information: We accept most HMO/PPO insurance plans, most types of Medicaid, and all Medicare patients. We are not an IHC facility. If you have questions about your coverage at our facility, please call your insurance company. The phone number should be listed on your insurance card.
Co-payments: Co-payments are determined by your insurance company and based on specific plan benefits. For most services, the co-payment will be listed on your insurance card. Co-Payments/Patient portions are due at the time of service.
Prior-Authorization: Authorization is a requirement of many insurance companies, and will vary based on the procedure. Ogden Regional will obtain authorization on all inpatient accounts. Your physician's office can assist you with pre-authorization.
Pre-Registration: Pre-Registration should be completed prior to the date of service in order to expedite the check-in process. General demographic and insurance information is required to complete the Pre-Registration. Refer to specific service to obtain more Pre-Registration information.

Inpatient Registration Information
Inpatients are patients staying in our hospital overnight. We receive inpatients through:
1. Surgical Admissions 2. Maternity Admissions 3. Direct Admissions (through the Emergency Room)
Surgical Admissions:
Scheduling:
Patients are scheduled for surgery by their physician's office. The Physician's office must call Ogden Regional's OR with information on the patient and the procedure.
Pre-Registration:
You may Pre-Register by telephone or face-to-face. Telephone: (801) 479-2004 Face-to-face: Report to our Pre-Op Registration in the hospital's main lobby.
Pre-Op Nurse:
All surgical procedures require a visit with the Pre-Op nurse. This must occur within 7 days prior to the procedure. The nurse reviews the medical history with the patient, discusses the procedure being performed and any preparations the patient should make. Any necessary Pre-Operative tests will be done at this time. To set up a Pre-Op appointment please call (801) 479-2004.
Day before service:
Day Surgery Unit staff calls the patient the day before service to inform them of the time they should arrive at the hospital.
Day of service:
Patients report to Day Surgery if they have gone through Pre-Op procedures. If the patient has not then they must stop at the Admitting Office with their physician's orders to complete the registration. Any co-pays, deductables or co-insurance required by your insurance should be paid on or before time of service, or talk with one of our financial counselors.
Maternity Admissions:
Pre-Registration:
Pre-Registration forms are available through our Main Registration Department, the Family Birth Place or through the patient's OB/GYN office. These forms gather basic demographic & insurance information of registration completion and insurance verification. Pre-Registration forms should be completed at the beginning of the second trimester or at the time you select Ogden Regional Medical Center for your delivery. Completed forms can be mailed into the facility or dropped at our Main Registration office.
Day of service:
When delivery date arrives, report directly to our Labor & Delivery floor (2nd floor Women and Children's Center). Please bring your insurance cards with you. Any co-pays, deductables or co-insurance required by your insurance should be paid on or before time of service, or talk with one of our financial counselors.
Outpatient Registration Information
Outpatient procedures are generally diagnostic in nature and do not require an overnight stay.
There are two registration locations: Central Registration (located at the hospital's main entrance) and the Emergency Department (Emergency Room). Each area is different, so please check your individual registration area.

Cardiac Rehab
Description:
The purpose of Cardiac Rehabilitation is to provide the recovering heart patient with the environment necessary to attain personal recovery and health maintenance goals.

Requirements for Registration:
Insurance card copy, valid photo ID, list of medications prescribed to patient including dosage, and a referral from Primary Care Physician or Cardiologist.
Special Requirements:
Cardiac Rehab treats patients based on an existing cardiac problem. If a recent cardiac procedure has been performed, our Cardiac Rehab unit needs to be informed of the Hospital, date of service, specific procedure, and attending Physician/Cardiologist involved in this procedure.
Educational Classes:
We offer education classes. These are open to Cardiac patients and the general public. Some classes have fees. Charges vary depending on the class. Call (801) 479-2286 and ask about our educational classes.
Emergency Room
Description:
The Emergency Room is the department responsible for the care of emergent illness and injury.
Location:
The Emergency Room is located around the West side of the building. Follow the signs from the front side of the hospital.
Hours of Operation:
The Emergency Room is accessible 24 hours a day.
Estimated Wait for Registration:
Registration procedures depend of severity of emergency. Some registration occurs bedside during treatment. In less serious situations, the registration will occur in the waiting room when the patient arrives.

Requirements for Registration:
For registration in the Emergency Room the patient must have an insurance card copy and a valid photo ID. Any ER co-pays required by your insurance should be paid at time of service, or talk with one of our Financial counselors.
Endoscopy
Description:
Department responsible for Endoscopic procedures including: scoping of the upper and lower digestive tracks, Bronchoscopy, Trans-Esophogeal Echo-Cardiogram (TEE), Liver Biopsy, Paracentesis and Thoracentesis.
Location:
North-West corner of Hospital on Main Floor.

7:00 am - 4:00 pm
Where to Register:
Register at the Admitting Office in the main lobby.
Estimated Wait for Registration:
10-15 minutes based on case load.
Estimated Procedure Time:
The patient can expect to be at the hospital for approximately two hours total.

Requirements for Registration:
Insurance card copy, Valid picture ID, Physician referral, Prior-authorization based on insurance. Any co-pays, deductables or co-insurance required by your insurance should be paid on or before time of service, or talk with one of our financial counselors.
Special Requirements:
Most procedures require a fasting period. Follow Physician's instructions. Refer any questions regarding procedure to ordering Physician. For any procedure, other than paracentesis and thoracentesis, the patient will need a driver to take them home due to the sedation required in the procedure.
Laboratory
Description:
Hospital-based Laboratory. Responsible for drawing and testing labwork for inpatient and outpatient treatments (walk-in).
Location:
Check with the Admitting Office to see if registration is required and they will direct you to the lab.

The average patient will have a wait of approximately 15-20 minutes based on case load and insurance of patient.

Requirements for Registration:
Insurance card copy, Valid picture ID, Lab order from Physician including diagnosis & type of tests ordered with Physician's signature on it. Any co-pays, deductables or co-insurance required by your insurance should be paid on or before time of service, or talk with one of our financial counselors.
Special Requirements:
Some labwork requires a fasting period. This depends on specific draw being performed. Refer to lab for fasting times if applicable. Follow Physician's instructions.
Pain Management
Description:
Pain Management is the department responsible for treatment of patients with Chronic Pain.
Location:
Enter through main entrance/lobby, and go to the Admitting Office. They will register the patient and give them directions to the Pain Clinic.

Call Pain Management Office to schedule appointment. On day of appointment register at the Admitting Office in the Main Lobby.

Radiology/Imaging
Description:
Performs radiology procedures for inpatient/outpatient and emergency treatment.
Location:
Enter the main lobby and go to the Admitting Office. They will register the patients there and give them directions to the Radiology/Imaging department.
Hours of Operation:
Radiology is available 24-hours for emergency procedures. For scheduled procedures they are available from 8:00am - 3:30pm.
Where to Register:
Radiology/Imaging patients must register at the main lobby admitting office.
Estimated Wait for Registration:
Please be ready to register in the Admitting Office 30-minutes prior to your scheduled appointment.
Estimated Procedure Time:
The estimated procedure time is 30-minutes (depending upon procedure and case load).

Requirements for Registration:
Insurance card copy, valid picture ID, physician orders including diagnosis and procedure. Authorization may be required by your insurance. Physicians can help patients in obtaining authorization. Any co-pays, deductables or co-insurance required by your insurance should be paid on or before time of service, or talk with one of our financial counselors.

Senior Health Center
Description:
Physician's office for patients over the age of 65 who have Medicare parts A and B.
Location:
The Senior Health Center is located in the Professional Building connected to Ogden Regional. The Center is on the second floor in suite 255.
Where to Register:
Senior Health Center front desk.
Hours of Operation:
8:00am - 4:30pm Mon-Fri (closed from 12:00pm - 2:00pm for lunch)
Estimated Wait for Registration:
15 - 20 minutes based on case load
Estimated Procedure Time:
Varies, similar to any physician's office visit.

Requirements for Registration:
Medicare card copy, secondary insurance card (if any), valid photo ID, and any medication patient is currently taking in bottles. The patient will need to fill out a functional assessment form required by Medicare.
